1. Description of Service

FASTA1C provides a blood glucose tracking tool that calculates an estimated A1C based on user-entered readings using the American Diabetes Association conversion formula. The Service is provided for general informational, educational, and personal wellness purposes only. It is not a medical device, does not provide medical advice, and is not a substitute for professional medical care or laboratory testing.

2. Acceptance

By creating an account or using the Service, you represent that you are at least 18 years of age and have the legal capacity to enter into these Terms. If you are using the Service on behalf of an organization, you represent that you have authority to bind that organization to these Terms.

3. Account Registration

You must provide accurate and complete information when registering. You are responsible for maintaining the confidentiality of your account credentials and for all activity that occurs under your account. Notify us immediately at privacy@fasta1c.com if you suspect unauthorized access.

4. User Responsibilities

You agree to use the Service only for lawful purposes and in accordance with these Terms. You are solely responsible for:

• The accuracy of any information you enter into the Service
• Verifying any output or estimate before relying on it
• Any decisions you make based on information obtained through the Service
• Maintaining appropriate backup copies of any data you export

5. Prohibited Uses

You may not:

• Use the Service for any unlawful purpose or in violation of any applicable law
• Attempt to gain unauthorized access to any part of the Service or its infrastructure
• Reverse engineer, decompile, or disassemble any portion of the Service
• Use the Service to transmit harmful, offensive, or fraudulent content
• Interfere with or disrupt the integrity or performance of the Service
• Enter false or misleading health information
• Use the Service on behalf of any individual under the age of 18

6. Health Information and Medical Disclaimer

THE SERVICE DOES NOT PROVIDE MEDICAL ADVICE. All estimates, trends, and outputs are for informational tracking purposes only. 7. Intellectual Property

All content, branding, design, software, and other materials comprising the Service are owned by FASTA1C or its licensors and are protected by applicable intellectual property laws. You may not reproduce, distribute, modify, or create derivative works without express written permission from FASTA1C.

FASTA1C and related names and logos are the property of their respective owners. Third-party names or marks referenced in the Service are used solely for descriptive purposes and do not imply affiliation, sponsorship, or endorsement.

8. Privacy and Data

Your use of the Service is also governed by our Privacy Policy and Consumer Health Data Notice, which are incorporated by reference into these Terms.

9. Disclaimers and Limitation of Liability

THE SERVICE IS PROVIDED "AS IS" WITHOUT WARRANTIES OF ANY KIND. TO THE MAXIMUM EXTENT PERMITTED BY LAW, FASTA1C SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF THE SERVICE. 10. Indemnification

You agree to indemnify and hold harmless FASTA1C and its owners, officers, employees, contractors, and agents from any claims, damages, losses, liabilities, costs, or expenses (including reasonable attorneys' fees) arising out of or related to your use of the Service, your violation of these Terms, or your violation of any third-party rights.

11. Termination

You may terminate your account at any time by deleting it in the Settings page. FASTA1C reserves the right to suspend or terminate your access to the Service at any time for any reason, including violation of these Terms.

12. Governing Law

These Terms are governed by the laws of the Commonwealth of Pennsylvania, without regard to its conflict of law provisions. Any disputes arising under these Terms shall be subject to the exclusive jurisdiction of the courts located in Allegheny County, Pennsylvania.

13. Changes to Terms

We may update these Terms from time to time. We will provide notice of material changes by updating the effective date and, where appropriate, by notifying you via email. Your continued use of the Service after any update constitutes acceptance of the revised Terms.

14. SMS Notifications Program

Program name: fasta1c glucose-reading reminders.

Program description: An optional SMS reminder service that sends you short text messages prompting you to log a blood glucose reading into your fasta1c account. Reminders are transactional notifications tied to the reminder schedule you select in Settings.

How to opt in: Sign in to fasta1c.com, verify your mobile phone number, then enable SMS reminders on the Settings page and choose a frequency. Opting in is a clear affirmative action; you may opt out at any time as described below.

Message frequency: Up to several messages per day depending on the reminder frequency you choose in Settings (off, daily, or multiple times per day). You can change or disable the frequency at any time from Settings.

Message and data rates: Message and data rates may apply. Standard message and data rates from your wireless carrier apply to every message sent or received. Contact your wireless carrier for details about your messaging plan.

HELP: Reply HELP to any fasta1c text message to receive support information. You can also email support@fasta1c.com for SMS support.

STOP: Reply STOP to any fasta1c text message to unsubscribe at any time. You will receive a final confirmation message and no further reminders will be sent. To resubscribe, sign in to fasta1c.com and re-enable SMS reminders in Settings.

Supported carriers:AT&T, T-Mobile, Verizon Wireless, Sprint, Boost, U.S. Cellular, MetroPCS, and other major U.S. carriers. Supported carriers are not liable for delayed or undelivered messages.

Privacy: Information collected through the SMS program is handled in accordance with our Privacy Policy. We will not share your mobile number or SMS opt-in status with third parties or affiliates for marketing purposes.
